How to Inspect or Request Your Medical Records
You have a right to inspect or review your records. You may also request a copy of your records to keep for your own use, to send to another doctor, or to forward to another healthcare facility.
Your medical records include your medical history, laboratory results, medications, physician reports from surgery, treatments, or other medical services administered at St. Vincent’s Medical Center or any St. Vincent's physician practice, urgent care center or clinic. We do not accept phone requests of your medical records.
Please review the following instructions for obtaining your health records.
Authorizing Requests for Your Medical Records
Patients or their legal representative may inspect or obtain copies of their St. Vincent's Medical Center medical records by first completing the Authorization for Disclosure of Protected Health Information (PDF), or Authorization for Disclosure of Protected Health Information (Spanish-PDF).
Indicate dates of treatment, especially if you've had multiple episodes of care.
Indicate what type of records you need.

Response to requests are generally processed within five to seven business days.
Cost for Copies
If you are requesting copies for your own personal use, you will be billed for charges ($.65 per page, plus shipping, in accordance with CT regulations). No cash will be accepted for payment of copies. There will be no charge for copies going directly to another medical facility or to another treating physician for continuing care.
Copies of radiology images
- Radiology images (films) are not part of your medical record, (i.e., your records contain reports of interpretations of such images). Please contact the Radiology Department to obtain copies of X-Rays or other films by calling (475) 210-5069. If you are looking for copies of images from the Breast Health Center please call (475) 210-5575.

Where to Bring Your Request
Please bring your completed Authorization for Disclosure of Protected Health Information form along with your photo ID to the Medical Center.
Health Information Services Department, Main Floor of the Medical Center. Hours: Monday–Friday, 8:00am–5:00pm (except holidays).
