Advanced Directives
Advanced directives can help you, your family, friends and doctors understand and know your decisions regarding treatment if you become seriously ill.
Your family members won't have to guess, and they won't have to make hard choices about your health care if you become so seriously ill that you cannot communicate your wishes. If you become too sick to make your own decisions about your medical care, your advanced directive will instruct your physicians and family members as to your wishes.
With advanced directives you can appoint a Health Care Agent with whom you share your decisions.
If you are interested in having a conversation about forming your advanced directives, please call the Department of Pastoral Care at (203) 576-5110.
