Your doctor or nurse will give you detailed discharge instructions for your post-hospital care. You will receive written discharge instructions. If you need assistance going over your discharge instructions, please ask your nurse for clarification. If you have questions about your diet, medication or activities, please speak up and ask questions.
The hospital does not provide discharge transportation. It's best to have someone available to drive you home after you are discharged from the hospital. You should coordinate with a family member, loved one or caregiver to arrange a ride home. Taxi cabs can be called on your behalf as well.
If you are being discharged after an outpatient procedure, you will not be allowed to drive yourself home and should make transportation arrangements ahead of time.
In most cases, your physician will prescribe one or more medications for you to use at home. To help with your transition we have an Outpatient Pharmacy that is open Monday to Friday 7am to 7pm. At St. Vincent's Outpatient Pharmacy, we can fill your prescription before you leave the hospital, saving you an extra stop on your way home. Please let your healthcare provider know as early as possible that you'll be using St. Vincent's Outpatient Pharmacy Services. That way, you can be sure your prescriptions will be ready when you are. Prescription refills can be transferred to your local pharmacy upon request.
