St. Vincent’s Club 50/Boomers Group

St. Vincent’s Club 50/Boomers is an organization for people age 50 and over who are interested in maintaining their health through educational programs and social events.

Geriatric medical services

Each stage of life brings different conditions, situations and expectations. At St. Vincent’s, our staff of board-certified geriatricians includes specialists who have expertise in areas of specific importance to older patients, such as orthopedics, nutrition and cardiology. They, along with all of our physicians, surgeons, gerontological and other skilled nurses, provide the kind of compassionate care and consideration that reflect our St. Vincent’s core values and mission.

In addition, St. Vincent’s offers a variety of other programs and services for individuals 50 and over. Though designed to meet very different needs, each is based on the recognition that all seniors want to live the most healthful lives they can. These include:

St. Vincent’s geriatric assessment:

Patients referred by a physician, family member or social worker are provided with a one-time geriatric assessment to evaluate their health needs. This evaluation is provided at the Geriatric Clinic located in St. Vincent’s Family Health Center.

Being alone while enduring worsening health is a fact of life for many of our elderly citizens. If you have a family member or a client who is not receiving medical care due to a physical, psychological or other barrier, St. Vincent’s Outreach program can help.

St. Vincent’s Senior Outreach program seeks to help those who are cut off from care because of physical isolation, disability/frailty, or lack of financial resources. Outreach provides primary care to low income, handicapped, homeless, frail elderly and homebound persons residing in Bridgeport. The 24-hour comprehensive primary healthcare and social worker services are provided by an interdisciplinary team including:

  • Physicians
  • Registered Nurses
  • Social Worker/Counselor
  • Outpatient/Registrar/Clerk

Accessing outreach services

To access outreach services, individuals must be:

  • Residents of the city of Bridgeport 
  • Unable to access other existing healthcare systems are provided with ongoing primary care through this program.

Guidelines:

  • The patient should have no actively involved primary physician.
  • The patient must be willing to accept ongoing primary care and agrees that Outreach is not a one-time emergency visit.
  • Patients must be willing to participate in the intake process so that information can be obtained about their current medical and social service needs in order to develop a plan for their care.
  • Patients must participate in a short interview conducted by the social service worker or registered nurse prior to being seen by a doctor.

Adult Medicine Center, located on the lower level of the Thomas Merton Center, 43 Madison Avenue, Bridgeport, CT   06604

Home Medical Visits (in the patient’s home)
Prior to the doctor visiting a patient’s home, qualifying patients must participate in a short interview conducted by the social service worker or registered nurse.

St. Vincent’s Parish Nurse Program:
Our Parish Nurse program provides nurse volunteers to numerous parishes and congregations in an inter-faith effort to bring health screenings, information and education to elderly area residents. Indeed, many of our St. Vincent’s nurses serve as health counselors, health educators & referral sources to members of their congregations.

Transportation

Some patients may be eligible for escorted wheelchair-accessible van transportation for medical services, if they cannot use existing community transportation.
