St. Vincent’s Regional Heart & Vascular Center offers a comprehensive program of self-care education and tools for people with Heart Failure.
Patients who cannot manage their heart failure condition through self-care alone can take advantage of St. Vincent’s Heart Failure (HF) Clinic. It provides frequent evaluation treatments and education especially for those patients with advanced heart failure.
Heart failure (HF) specialists
Patients are supported by a team of specially trained registered nurses, nurse practitioner, pharmacists, dieticians, and palliative care specialists that work in conjunction with the patient’s cardiologist to provide the best plan of care taking into consideration the patient and family's own personal health care goals.
The HF program at St. Vincent’s is proactive rather than reactive, seeking to give patients an improved quality of life with fewer hospital admissions, shorter hospital stays and the availability of technology for patients with HF who can stay home and remain independent as long as possible.
Additionally, the HF program will help to facilitate a more palliative approach and transition to hospice when appropriate.
