Referrals come from physicians, nurses and other members of the multi-disciplinary team and also via patient request.

Cardiac Rehab Phases

At St. Vincent’s, we partner with patients through three phases of rehab with the goal of returning them to healthy and active lifestyles.

Phase I –

Phase I begins in the hospital after you have undergone cardiac surgery or experienced a heart attack or other heart problem. The inpatient phase focuses on patient education and ambulation or walking assistance, and helping you determine how well you can care for yourself. Both ongoing patient and family education will be provided regarding the importance of a healthy diet and lifestyle habits. Your ability to exercise will also be assessed as a part of Phase 1.

Phase II –

Approximately four to eight weeks after surgery or an acute event,  you will begin Phase II, which is a12 week nationally-certified AACVPR outpatient program consisting of exercise, education, stretching, relaxation and counseling.  Classes are held three times per week for 1.5 hours each and are offered in the morning and late afternoon.

The goal of the program is to optimize your  recovery and assist in rebuilding your  confidence and functional independence. Patients progress from a nervous and sometimes vulnerable frame of mind to a more secure state of well-being both physically and  emotionally. St. Vincent’s staff are on hand to answer all your questions, so patients leave the program self-reliant and confident with a new outlook on life.

Phase III –

Patients embark on a cardiac wellness/maintenance program when they complete Phase II. Phase III helps them establish and maintain a lifelong healthy lifestyle regarding diet, exercise, stress reduction and medical compliance. Some patients have remained in the program for more than 10 years, because it is often the link that keeps them on the wellness track. These classes are offered daily at various times. Phase III can also be utilized by patients with a physician’s order for non-cardiac diagnoses, but who require support and education about general wellness.
