Medicare Screening Criteria
Patient requires:
- close supervision of a rehab physician
- 24 hour nursing care
- intense rehab services - at least 3 hours per day, for 5 days per week
- requires multiple therapies/ services
- a reasonable expectation of improvement
SVMC Admission Screening Criteria
- medically stable
- needs close medical supervision
- requires 24 hour nursing care
- functional deficits present
- requires multi-discipline treatment
- able to tolerate 3 hours per day of therapy
- reasonable goals in a reasonable time frame
- safe, viable discharge plan
- approved by insurance carrier
Medicare-Approved Diagnoses
(75% of patients must have one of these as their primary diagnosis)
- 1. Stroke
- 2. Spinal Cord Injury
- 3. Congenital Deformity
- 4. Amputation
- 5. Fracture of the femur (hip fx.)
- 6. Brain Injury- tumor, meningitis, abscess, anoxia, SDH
- 7. Neurological Disorders- MS, Parkinson’s, Guillian Barre, etc.
- 8. Burns
- 9. Polyarthritis, including Rheumatoid Arthritis (if outside treatment has failed, and MD has documentation.
- 10. Systemic Vasculitis with joint inflammation
- 11. Severe or advanced osteoarthritis
- 12. Hip or knee joint replacement
- A) bilateral
- B) unilateral-age 85+
- C) unilateral-BMI > 50
13. Major Multiple Trauma: - brain and spinal cord,
- brain and multiple fractures/amputation,
- spinal cord and multiple fractures/amputation, or
- multiple systems or sites affected that would justify Acute Rehab admission (such as liver laceration and femur fracture.)
