Medicare Screening Criteria
Patient requires:

• close supervision of a rehab physician
• 24 hour nursing care
• intense rehab services - at least 3 hours per day, for 5 days per week
• requires multiple therapies/ services
• a reasonable expectation of improvement

SVMC Admission Screening Criteria

• medically stable
• needs close medical supervision
• requires 24 hour nursing care
• functional deficits present
• requires multi-discipline treatment
• able to tolerate 3 hours per day of therapy
• reasonable goals in a reasonable time frame
• safe, viable discharge plan
• approved by insurance carrier

Medicare-Approved Diagnoses
(75% of patients must have one of these as their primary diagnosis)

• 1. Stroke
• 2. Spinal Cord Injury
• 3. Congenital Deformity
• 4. Amputation
• 5. Fracture of the femur (hip fx.)
• 6. Brain Injury- tumor, meningitis, abscess, anoxia, SDH
• 7. Neurological Disorders- MS, Parkinson’s, Guillian Barre, etc.
• 8. Burns
• 9. Polyarthritis, including Rheumatoid Arthritis (if outside treatment has failed, and MD has documentation.
• 10. Systemic Vasculitis with joint inflammation
• 11. Severe or advanced osteoarthritis
• 12. Hip or knee joint replacement
  • A) bilateral
  • B) unilateral-age 85+
  • C) unilateral-BMI > 50
  13. Major Multiple Trauma:
  • brain and spinal cord,
  • brain and multiple fractures/amputation,
  • spinal cord and multiple fractures/amputation, or
  • multiple systems or sites affected that would justify Acute Rehab admission (such as liver laceration and femur fracture.)
