Community Health Care at St. Vincent’s

Patients are seen at the FHC by appointment, which can be scheduled by calling Monday through Friday between the hours of 8:00 a.m. and 4:30 p.m.
Telephones are busiest between 9:00 and 11:00 a.m., and again between 1:00 and 3:00 p.m. To avoid long wait times, you may wish to call earlier in the morning or after 3:00pm
Blood work is performed for the FHC patients on site at the FHC; Monday through Friday, 8:30 a.m. to 4:30 p.m. No appointment is necessary.
Appointments to see one of our specialists can be made after getting a referral from one of FHC’s medical providers.
Patients are requested to arrive 15 minutes before each scheduled appointment.
A photo ID and your insurance card, if applicable, are required at every visit. In addition, patients may be required to bring a minimum payment of $25.00 at the time of appointment.
Interpreter services, including sign-language interpretation for the hearing impaired, are available at the FHC.
