St. Vincent's has been on a journey to become a Highly Reliable Organization, a new approach to patient safety. The Medical Center is partnering with Healthcare Performance Improvement (HPI) over a two-year period in its effort to create error prevention tools and programs and achieve a standard of zero preventable errors.
With the help of HPI, St. Vincent's will learn about high reliability from organizations outside health care that have made their business safe. HPI performed benchmarking at St. Vincent's to determine targeted areas in need of the most improvement, and is also quantifying the 'failure modes" behind each error, including lapses in critical thinking, compliance or lack of competency.
